This article tells the hopeful story of Collaborative Inquiry, a strengths-based approach to inquiring into child deaths to search for hat works even when the worst happens. It looks, without blame, for the how and the why in the whole system which allowed, unwittingly and unknowingly, a series of errors to happen that led to catastrophe.
